Care Coordinator

Overview
To be responsible for coordinating the care of patients accessing assisted conception treatments via NOW-fertility, and be part of a global multidisciplinary team embracing technology solutions and innovative measures to offer an unparalleled experience for patients.
Key Responsibilities
To conduct virtual initial advisory appointments with patients using any digital and/or remote solutions, including but not limited to the NOW-fertility secure cloud-based platform, with the primary objective of:
- Introducing NOW-fertility and explaining the benefits of the programme
- Ensuring GDPR and any other data protection forms are duly completed
- Reviewing the enquiry forms Collecting additional information/history, as necessary
- Offering/organising appointment with physicians and nurses, as necessary
- Creating patient file and assigning the patient unique identifying number for basic and full access
To conduct virtual follow-up appointments with patients after the initial appointment with the physician, using any digital and/or remote solutions, including but not limited to the NOW-fertility secure cloud-based platform, with the primary objective of:
- Reviewing the individualised treatment plan recommended by the physician
- Explaining the next steps
- Presenting and discussing the costs and package options
- Arranging all screening tests, if required
- Arranging the collection or delivery of medications, as recommended by the physician (according to the electronic prescription), and if arranged via NOW-fertility
- Signing the treatment agreement
- Completing the treatment location form
- Contacting the recommended Centre of Excellence
- Confirming with the patient availability at the Centre of Excellence
- Sending cycle’s unique access link to the Centre of Excellence to access patient records
- Introducing patient educational resources
- Answering any queries
To introduce NOW-fertility finance and legal services, once these are available
To create and maintain up to date the list of Satellite Clinics in each country
To ensure a smooth flow of communication and work between NOW-fertility team and patients
To ensure a smooth flow of communication between NOW-fertility team and Centres of Excellence / Satellite Clinics
To assist patients with making any appointments either in-person or virtual with the NOW-fertility clinical team or at the Centres of Excellence or at the Satellite Clinics, as required
To assist the NOW-fertility physicians, nurses and admin staff as necessary
To work as part of a team in a rota system to provide continuous advice and support to patients 24/7 using any digital and/or remote solutions
To arrange blood pregnancy tests and/or follow up appointments with the physician
To agree date of viability ultrasound scan with the chosen Centre of Excellence, and arrange the ultrasound scan
To ensure the Centre of Excellence team enters all the clinical and laboratory information in the patient’s electronic medical file
To obtain patient feedback and testimonials at designated points during the treatment process
To contribute to educational and training resources, including printed material and video recordings for patients
To contribute to blogs and articles for the website, social media and communication/PR
To participate in ongoing quality management including audits and risk assessments
To ensure compliance with legislative requirements in relevant countries and organisational codes of practice
To handle patient data responsibly at all times and be compliant with mandatory training
To work collaboratively with internal and external colleagues, treating everyone fairly and equally and encouraging and embracing diversity within the team
To participate to in-person and/or virtual patient’s events, meetings and forums, as required
To represent NOW-fertility at events, meetings, forums and congresses, as required

Qualifications & experience
Excellent customer care skills – (essential)
Previous experience in healthcare, fertility, women’s health or pharmaceuticals – (essential)
IT literate – (essential)
Fully conversant and comfortable with the use of virtual platforms to communicate with patients – (essential)
Ability to speak additional languages – (desirable)
